Using Insurance:
As a member of Shelterwood Collective I provide in-network mental health services with Premera (Blue Cross) and Lifewise contracts.
Although I am able to provide some insurance verification and confirmation of benefits information, you are ultimately responsible for identifying and undersigning your unique insurance coverage. If you are choosing to work with insurance for mental health care I recommend reaching out to them directly to determine your coverage.
Asking the following questions can help you understand your coverage when calling your insurance company:
What are my mental health benefits?
Does a deductible apply?
(If yes) How much is my deductible?
What is my copay (or coinsurance)?
Does my plan have any special coverage for telehealth office visits?
Who processes benefits for claims submitted in Washington state? (Regence or Premera)
My therapist is a member of a group practice, Shelterwood Collective. Their tax # is 47-4860247 and their NPI-2 is 1538663604. Can you confirm that this group is in network with my plan?
Using Out-Of-Network (OON) Benefits:
If you anticipate paying for therapy out of pocket, I suggest contacting your insurance company prior to beginning therapy to check if your plan offers out-of-network (OON) benefits. If your plan does include these benefits, you'll pay for therapy services upfront, but I can supply you with a Superbill containing all the details needed to submit to your insurer for partial reimbursement.
Asking the following questions can help you understand how much you should expect to pay out of pocket:
How much do you reimburse for CPT code 90791 (for an intake assessment) and 90837 (for ongoing individual counseling) if the provider is OON?
Do I have a deductible to meet? If so, how much?
How much can I expect to pay per session after I meet my deductible?
Is there a limit on the number of sessions Iām allowed?
How quickly will I receive reimbursement after submitting a superbill?
